That right side (our left) looks like it has another deer's antler wedged into it. I see the base of an antler and an eye guard sticking out there.

Not fake. you can "watch him grow" by pictures at www.farmingforwildlife.com and click on "watch him grow" in the upper left of the page.

Pretty neat, I have been keeping tabs to see if he gets much bigger. You can also see how he progressed last year.
