All good questions. They are probably running off of satellites I'd imagine, otherwise they'd be no different than cell phones.

Actually not "has anyone used one". 
It looks like it is cellular, and it really doesn't answer whether this is truly "nationwide". The middle of the frank church wilderness is a long way from any cell towers. I can't understand how it would work in this or similar situations and looking at the "coverage map" there could be huge holes in Idaho and other areas as well as most of Alaska.
