USFS Special Use Permit

What is involved in obtaining a Special use Permit? Is it just a matter of filling out the paperwork or do you have to make a presentaion and convince them that it is a worthwhile venture?

I know they take a certain percentage of your gross earnings and require an insurance policy, but does everyone with a legit request get granted a permit?
