I see that the mobile disability form has only mention of wheelchair / walker. Can someone with a doctors order of upper body disability apply for crossbow use??

Yes, if it’s a permanent disability you may get a note from your physician and be approved for a mobility impaired number.
If it’s a temporary injury or surgery of some sort you may also apply for a reasonable accommodation for the hunt you’ll need it for.

They have got way more strict with their approval of mobile impaired. For mobile impaired hunts anyway. Not sure if that is different than getting crossbow approval.
