Transferring my tag to my grandson

I'm looking for info on what I need to transfer my tag to my grandson. Besides the tag and his hunting license, what else do I need to prove that I am his grandfather to complete the tag transfer?

I called the office with no real luck getting a hold of someone to answer my questions. Hoping someone here might know.

If he is under 14 a hunter ed course needs to be completed. They made my wife sign an affidavit that she was the grandparent, her original tag and an $8 transfer fee. I may be forgetting something but I think that was it. Pretty simple.
